-Gather all the necessary documents and tests you have had in the past that may be useful to your doctor to better understand your condition.
- Copies of previous tests, x-rays or medical history related to your medical problem.
- Information about your medical history.
- Alist of medications you take on a regular basis, including over-the-counter medications.
- A list of allergies you may have.
- Follow any instructions from your doctor about diet or medication before your appointment.
- Ask a relative to accompany you, if you wish. Depending on
the appointment you have, you may need someone to return you home.

What
to expect after your appointment
If you need to see another doctor or have an additional test, we'll try to schedule it at the same visit so you don't have to go through the hassle. If this is not possible, we will schedule your next visit as soon as possible.
If you need to be hospitalized, we will tell you about all the necessary steps to take.
